* The student rate is for those who have not yet received a Ph.D. You must include a copy of your student card or statement from your supervisor to verify you are a student. ** It has come to our attention that it can take as long as six months for some visa applications to be processed. Please notify us now if you require a letter for early entry visa application. *** The reduced workshop rate is available only if you also register for the conference

Conference rates include Thursday opening champaign reception and Saturday Midwest cookout (barbecue)

Additional Information:
• The registration fees do not cover insurance, travel, or accommodation.
• Travel insurance is recommended by the organizers.
• People who must cancel their registration are eligible for a full refund up until April 6, 2005
• A $25 fee after will be charged for cancellations received April 7 up until April 21, 2005
• After April 21, there is no refund available
• Substitutions are allowed any time
• Online registration ends May 1, 2005
• Limited conference fee waiver info

Note: Any participant unable to attend for no fault of their own (e.g. visa complications, serious illness, etc.) will receive a full refund upon request regardless of their cancellation date.
